ANONYMITY: Even if you submit names and your own contact details, the data collected will be treated with anonymity.

This form and the information collected is hosted by the British Stunt Register in the interests of industry-wide research and development.

Submitting the report more than once could affect the significance of the data when it is reviewed.

As you are actively encouraged to use this form we realise that multiple reports about the same incident will be submitted, and because of that we ask you to share as much information as possible including accurate dates, locations and production names.
